Update: COVID-19 Vaccine for Seniors

Cook County’s Department of Public Health (CCDPH) understands that many people, especially older adults, want to get vaccinated for COVID-19 as soon as possible. Governor Pritzker announced that Illinois will move to phase 1b – which includes senior adults aged 65 and older — on January 25.

The initial step to be considered for vaccination is to complete and submit an online form that will place your name on Cook County’s “notification” list.

Cook County will then notify you, at a later date, when and how to make an appointment to receive the vaccine.

Please note: Completing the “notification list” form doesn’t guarantee immediate vaccination. It just places you on the list for consideration.

HOW TO ADD YOUR NAME TO THE “NOTIFICATION” LIST:

  1. Visit CCDPH’s website: http://bit.ly/3bRE4DW to go to the Vaccination Survey Form
  2. Scroll down to where you enter your information.
  3. Provide your information.
  4. Hit the “Submit” button at the bottom of the form.
